b)Print out and sign the DELIVERY RECEIPT (“Protocolo”) by visiting the same web site and clicking “PRINT VISA FORM DELIVERY RECEIPT” on bottom left-hand side of screen.

3) One photograph

  • Recent, not older than 6 months, in color, full-face only, on a white or clear background. ‘Selfies’ are unacceptable.

4) Round-trip-ticket

Applicants must present one of the following items:

  • A hard copy of the round-trip-ticket or e-ticket bearing applicant’s name.
  • A signed letter from the airline or travel agent, on their letterhead, with the itinerary, name(s) of the passenger(s), flight number or cruise information, and dates of arrival and departure from Brazil.
  • For airline employees and their dependents: a copy of their airline I.D. card and a letter from the airline, on company letterhead, with the itinerary.

5) Proof of residence in consular jurisdiction for applications by mail only

A photocopy of a valid driver's license or utility bill will be accepted as proof of residence.

6) Consular Fees

The Consulate General in Chicago ONLYaccepts payments by Money Order issued by the United States Postal Service (USPS), payable to “Consulate General of Brazil in Chicago”. Personal checks, credit cards, or any other forms of payment are not accepted.

Multiple applications submitted together may be covered by one single money order for the total sum provided the payment is being made within the same department. When submitted by mail, multiple applications may be covered by one single money order only if they are sent in the same U.S.P.S. envelope.

7)International Certificate of Vaccination

Applicants who have visited certain countries and territories (view list here) within 90 days prior to applying for a visa/entering Brazil are recommended to present a Yellow Fever International Certificate of Vaccinationupon entering Brazil.

8) Letter by Sponsor or Invitation Letter

  • (Applicable only for participants in amateur athletic, scholarly, or performing arts events and/or competitions that will result in income arising from a Brazilian entity for their activities.)
